  1. Complete a Priority Health application (432KB PDF).
  2. Download and complete the ORS insurance enrollment/change request form (157KB PDF) OR log in to your miAccount on the ORS website, fill out the change form and print it. (Be sure to keep a copy for yourself.)
  3. Return the application and enrollment/change forms to:

Help save paper

Is there more than one person in your household with the same Priority Health Medicare plan? Help save paper by requesting that we mail only one copy of identical materials (such as your Annual Change Notice) to your home.
